Site plan approval by the Planning & Zoning Commission for the wireless telecommunication facility including access, equipment, and structures shall be in accordance with the site plan regulations specified within this zoning ordinance. B. All applications for approval of wireless telecommunications facilities shall include: 1. A description of the need for the wireless telecommunication facility including propagation studies showing service area and system coverage in the Town and surrounding area. 2. Sufficient information to indicate that construction, installation, and maintenance of the wireless telecommunication facility will not create a safety hazard or damage to the property of other persons. 3. Documentation of the area to be served including a search area for the antenna location. A narrative describing a search area (with not less than a 1 1⁄2 mile radius) clearly explaining why the site was selected including documentation that all potentially suitable existing or approved towers, structures or buildings within the search area were considered for the communications equipment, and if rejected, the basis for these conclusions. 4. A copy of all reports including the environmental assessment, National Environmental Policy Act review, etc. as required by or provided to the Federal Communications Commission. 5. Photographs of the existing conditions of the site and photo simulations of the proposed wireless telecommunication facility installation from at least four directions and from no more than 1⁄4 mile, including from residential properties within that distance from which the facility would be visible. 6. For applications for new towers, photo documentation of the balloon test that was conducted at the proposed site location, sufficient to show the visibility of the tower from affected residential areas. This test will be conducted by raising upon a tempor ary mast, a 3-foot diameter minimum, brightly colored balloon at the precise location and maximum height of the proposed tower. C.
